What do I do if I need a referral?

If your insurance company requires a referral, please have your primary care physician forward the referral to us. We can also contact your physician at the time of your initial appointment.

How do you handle payments and fees?

If you are not covered by a health insurance plan, payment is due for an office appointment at the time of service. Financial arrangements need to be made prior to surgery being scheduled. Cash, check, Mastercard, Discover, and Visa are accepted as forms of payment.

Which insurance plans do you accept?

We accept most health insurance plans. We will file a claim with your insurance company for services provided to you; however, co-pays or deductibles are due at time of service. Please be prepared to pay this amount. A co-payment is a type of cost sharing whereby insured persons pay a specified flat amount per service. A deductible is the amount you must pay before insurance begins to pay for services. Please bring your insurance card to your appointment.
